Major Medical
Monument Chiropractic is a provider for most major medical insurance companies.   It is our standard procedure to complete the proper paperwork for our patients so that we will be reimbursed directly from the insurance company.  We will also research your policy to determine you proper co-pay. 

Please note that we are not providers for:

• Kaiser

• Tri-Care

• Blue Cross/Blue Shield

If you have one of these providers, your office visit will need to be paid for at the time of service.

Medicare
Monument Chiropractic is not a Medicare provider.  If you have Medicare, your office visit will need to be paid for at the time of service.  We will submit all charges to Medicare for them to evaluate your reimbursement.

Personal Injury / Auto Accidents
Monument Chiropractic will work with your attorney and/or insurance company directly, therefore eliminating out-of-pocket-expenses for you until your case is closed.  Most Auto Policy Med-Pay covers expenses for chiropractic rehabilitation.

Workmen’s Compensation
Please check with your employer for authorization to receive treatment at this facility before making your appointment.  If authorized, it is our standard procedure to complete the proper paperwork for our patients so that we will be reimbursed directly from the insurance company.
